Christ Is Made the Sure Foundation → Blessed city, heavenly Salem – The hymn "Christ is made the sure Foundation" is originally a component part of the larger work, the hymn "Blessed city, heavenly Salem", both being translated from the same source Urbs beata Jerusalem dicta pacis visio. The two hymns often appear separately in modern hymnals, but should be understood as part of the same work. Also, I think this capitalisation is correct per MOS:INCIPIT.
