Add Issue tempalte for docs issues

There are now buttons for creating issues from each page of the docs so this is mostly done. The only remaining work is to add a Issue template here and update the "edit this page" links to use that template.

It would be good to update ParameterExpression.__eq__ to accept comparing a bound expression to a fixed value, but this can be a separate issue.

go-jwt seems to not be maintained really. It has open/known security issue (which have multiple PRs open to address, earliest in 2018: dgrijalva/jwt-go#286). And it requires numbers to be floats because it cannot validate ints. [PR to fix this has been made](https://github.com/dgrijalva
