Regarding the Sign Out use-case, it would be nice to include the User's display name with the "Sign Out" text.

Should this be a separate issue?

Looks like they're doing themes already. #1584 That may be a good place to mention this too.

i would like it even better if there was an option in the rules to specify a path to the desired image for each rule. that way you could specify the image after the web app you want to protect.
