JUDICIAL AFFIDAVIT

Purpose of the testimony: Plaintiff, THIRSTY R US is presenting the


testimony of LOLITA SURE, to prove that VICENTE ABAD tendered
his resignation last September 1, 2019.

LOLITA SURE, of legal age, Filipino, the Human Resource


Department Head with office address at 5th floor, TRU building,6th
Lacson Street, Bacolod City, after having been sworn in accordance
with law and fully conscious that I do so under oath and that I may
face criminal liability for false testimony or perjury in way of answers
to the questions propounded to be by Atty. Kristel Ann Develos, with
office address at 7th floor, MT3 building, Lacson street do hereby
depose and state:

Q1: Please state your name and other personal circumstances for the
record.

A: I am Lolita Sure, 39 years old, HR Head of TRU Corporation for 7


years now.

Q2: Ms. Witness, as the HR Head at TRU corporation, what are your
duties and responsibilities?
A: I plan, direct, and coordinate the administrative functions of an
organization. Also, I oversee the recruiting, interviewing, and hiring
of new staff; consult with top executives on strategic planning; and
serve as a link between an organization's management and its
employees.

Q3: Is part of your job accepting resignation letter?

A: Yes, it is also part of my job.

Q4: Have you received any resignation letter?

A: Yes, I have received a resignation letter.

Q5: From whom?

A: From Mr. Vicente Abad

Q6: How do you know Mr. Abad?

A: Mr. Abad was the former Accounting head of TRU Corporation.

Q7: To the best of your knowledge, do you remember when did he


tender the resignation?

A: It was in the month of September.

Q8: I am showing to you the resignation letter of Mr. Vicente Abad. Is


this the letter you have received last September 1, 2018. (Counsel
handing over the document)

A: Yes Ma’am, that’s the letter that I have received when Mr. Abad
decided to tender his resignation.

Q9: How did you know that he is Vicente Abad?

A: When he tendered the said resignation, he delivered to me


personally.

Q10: After receiving the resignation letter, did you do anything?

A: As per company protocol, I issued a HR memorandum posted


within the premises of the company announcing that Mr. Vicente
Abad was no longer connected to us. Also, to give notice to the
general public, the company published on a newspaper of general
circulation the same notice.

Q11: Ms. witness, I am showing documents, can you identify to this


court what are these? (Counsel handing over the documents)

A: The documents are the HR memorandum I issued and the


publication we posted in a newspaper of general circulation last
September.
