PUSKESMAS MODOPURO

1. History
This Puskesmas was established in 1975, but in that time this place is not Puskesmas
but still auxilary health center (Pustu). An then in 1985 it become Puskesmas
officially. In 1985, it still has 4 staff: Head of Puskesmas, Midwife, midwife aide, and
administrator. Years by years the staff here is increasing and until now there are 42
staff.
This puskesmas has an area of 15.211 km2. with boundaries:
a. North : Subdistric Panggon in Distric Sidoarjo
b. East : Subdistric Pungging in Distric Mojokerto
c. South : Subdistric Kutorejo in Distric Mojokerto
d. West : Subdistric Bangsal in Distric Mojokerto
There are 8 Villages in Modopuro
a) Modopuro
b) Pekukuhan
c) Kebon dalem
d) Menanggal
e) Jotangan
f) Kedung gempol
g) Ngimbangan
h) Leminggir

2. Motto and Vission and Mission


Motto: “MDP”
M: Independence
D: Dynamic
P: Professional

Vision:
Realization of the Modopuro community Health Center working area in
healthy living independently.

Mission:
■ Improving Community Health Degree
■ Encouraging Community Independence for a Healthy Life
■ Improve the Equal and Affordable Community Health Services

3. Facility
Ambulance, laboratorium room, dentistry clinic, nutrition room,
administration room, emergency room, and pharmacy. In this puskesmas houses 31
Posyandu, 2 auxilary health center and 5 poskesdes.
4. Service
1. Dental examination
Here they would examine your dental health
2. Laboratory examination
Here, you can examine your blood, the content of your blood or blood check
3. Emergency room
This service is specially for people that need immediate treatment, or people that
in critical condition
4. Receive services for the general public or BPJS.
This service is for patient that using BPJS
5. Maternal and family planning health services.
Service for mom and kids
One of the program from this puskesmas just actively do is promotion in one village
called Leminggir that almost all the people there still using river beside their village
as a toilet, staff in this puskesmas promote people there to less using river as a toilet.
