Attorney’s Office through the undersigned Public Attorney, unto this
Honorable Court most respectfully avers that:

1. Accused availed of plea bargaining and pleaded guilty to the


lesser offense of ___________________________________;

2. The case proceeded for judgment and the accused was


sentenced to suffer imprisonment and payment of fine in the
Order of this Honorable Court dated
______________________________;

3. Accused wishes to apply for probation for the imposed penalty


yet to be served;

4. Accused has not been convicted by final judgment of any other


crime, thus, qualified to avail of the probation program;

5. The offenses for which the Accused was convicted is not a crime
against the Security of the State and Public Order, and he does
not suffer from any of the disqualifications of Probation Law
and has not been previously placed on probation;

6. Accused is sincerely remorseful of having committed the offense


and if granted probation, he is willing to undergo supervision
under such terms and conditions that may be imposed by the
Honorable Court;

PRAYER

WHEREFORE, premises considered, in the interest of justice and


for humanitarian consideration, it is most respectfully prayed upon this
Honorable Court that this Application for Probation be considered and
granted.

Other reliefs just and equitable are likewise prayed for.
