This form is to verify the Company’s election to provide Employee with a monthly allowance for internet
service while Employee is on a work-from-home arrangement.

Please complete the information requested below and submit to Human Resources for record keeping
purposes.

I understand that the Company is providing this internet allowance as a necessary tool of trade for all
employees who are on a work-from-home arrangement, and that I will use it to promote efficiency in
the performance of my work roles and responsibilities.

I certify the acceptance of the above allowance and understand that the continual receipt thereof is
contingent on business need, and subject to regular review. Given that this internet allowance is not
considered an additional compensation or benefit, it may be adjusted or completely removed by
management at any given time, depending on business and operational needs, and upon the sole
discretion of management.

By signing this Conforme, I acknowledge that agree to receive the internet allowance to be provided by
the Company, subject to the above terms and conditions for the use thereof.
