Mamata Scheme AWC Monthly Report: New Registrations

1) This document is an AWC monthly report form collecting information on beneficiaries of the Mamata Scheme in a particular month, including new registrations and beneficiaries receiving installment payments. 2) It collects details of new beneficiaries registered, beneficiaries sponsored for the first and second installments, and beneficiaries exiting without receiving installments. 3) For each category, it requires information like beneficiary name, ID numbers, amount due, reason for exit, and whether required documents are attached, to be submitted to the Child Development Project Officer (CDPO).
