Seacare Medical Scheme (SMS) – FOREIGN

SMS Philippines
The Seacare Medical Scheme (SMS) for SOS Filipino Members is implemented on 1 March 2005 by the
Singapore Organisation of Seamen (SOS), as a special medical scheme to provide free medical
consultation, basic medical examination and tooth extraction to SOS Filipino members and their
dependents at the affiliated clinics of the Seacare Thrift Pte Ltd in Philippines.

i. Eligibility
To be eligible, the SOS Filipino Member must be serving on board a vessel covered by the SOS Collective
Bargaining Agreement and whose employer is participating in the SMS.

A SOS Filipino Member who meets these criteria will be termed as a Qualified SOS Filipino Member.

For Dependent

Dependents of Married Qualified SOS Filipino Member refer to:

• Legitimate wife
• Unmarried legitimate children below 18 years of age

Dependents of Single Qualified SOS Filipino Member refer to:

• Natural parents or, if legally adopted, legal parents


• Unmarried legitimate siblings below 18 years of age

ii. Benefits & Conditions


A. Free Benefits
A Qualified SOS Filipino Member and his dependents are entitled to receive free medical benefits at the
affiliated clinics of the Seacare Thrift Pte Ltd in Philippines.

The medical benefits are as follows:

1. Medical Consultation
2. Complete Blood Count (CBC)
3. Stool Examination
4. Urinalysis
5. ECG
6. Chest X-Ray
7. Fasting Blood Sugar (FBS) Check
8. Total Cholesterol Check
9. Pap Smear

The dental benefits are as follows:

1. Dental Consultation
2. Tooth extraction without surgery (2 tooth extractions per person per month)
3. Simple tooth filling (2 tooth fillings per person per month)

The free medical and dental benefits are subject to the following conditions:

• Each family, comprising of the Qualified SOS Filipino Member and his dependents, is entitled to a
maximum of 4 visits per calendar month; and
• Any unconsumed entitlements are not allowed to be carried forward to the following month or
exchanged for cash

B. Reimbursement Benefit
The reimbursement benefit is subject to the following conditions:

1. Each medicine reimbursement is as per amount stated on the receipt or up to Php 500 per person
per claim.
2. Each family, comprising of the Qualified SOS Filipino Members and dependents is entitled to 4
medicine reimbursement benefits per month limited to 1 reimbursement per person per month.
3. Reimbursement must be transacted within 14 days from date of consultation.
4. Only prescription issued by SMS appointed clinic will be accepted for reimbursement.
5. Any unconsumed entitlements are not allowed to be carried forward to the following month or
exchanged for cash.

iv. Procedures
1. A Qualified SOS Filipino Member is required to complete a Nomination Form which is obtainable
from the Crewing Agent or Employer. The completed Nomination Form must be submitted to SMS
Liaison Officer through the Crewing Agent or Employer.

2. A Qualified SOS Filipino Member will be notified by post via his last registered address declared
on his Nomination Form once it is approved.

3. A Qualified SOS Filipino Member and his dependents must produce either one of the following
documents at the affiliated clinics for verification:

a) Birth Certificate
b) Passport
c) School Identification Card
d) SSS ID
e) Voter's ID

A Qualified SOS Filipino Member and his dependent are advised to bring along the Notification Letter to
the clinic, in case required to be produced.
