Memorandum of understanding

MOU FOR OUTSOURCED LABORATORY

Between

GURU NANAK MEHAR HANS EYE HOSPITAL (1ST PARTY)


situated at Railway Station – Gurudwara Dukhniwaran Sahib Road, Near Hotel Flyover, Patiala,
Punjab - 147001

And

DR. DINESH PAH LABS (NEW KAMAL LABORATORIES) (2ND PARTY)


D.P Diagnostic Complex, New Leela Bhawan, Adj. to Income Tax Office, Patiala, Punjab – 147001

On date 05/03/2024 for 5 Years.

1. The 2nd party is running Pathology laboratory.


2. The 2nd party will obey the rules and regulations of Government of India, respective
State Government, State Medical Council, Para Medical Council Of India and NMC
3. The 2nd party will provide services to 1st party 24 hours and during emergency.
4. The Patient will pay directly to 2nd party and 2nd party will issue proper receipt to patient.
5. The 2nd party will allow the 1st party to visit the premises to audit the working and
quality. If any discrepancy is found in this than the 2nd party will have to take corrective
action against it at the earliest possible, else the MOU will stand cancelled.
6. The 2nd party will take necessary precautions while handling blood samples so that
workers are not exposed to any risks.

7. The Lab shall ensure it has a requisite mix of trained and qualified staff as needed to
perform and assist in all tests.
8. The laboratory shall not refuse services on any grounds.
9. 2nd Party shall provide the hard copy and typed report to all the patients with in the
stipulated period, and not charge anything extra apart from the package deal from the
hospital or patients.
10. The 2nd party shall inform the first party immediately whenever there is any result which
is critical for any patient referred to it by the 1st party. These shall be intimated to the
referring consultant and Nursing Superintendent.
11. The 2nd party shall make sure that it will try to follow the turnaround time it has stated
for the tests.
12. If any of the parties in the MOU do not want to continue the MOU than they will have to
give a month’s notice to the other party. After a month’s notice the MOU shall stand
terminated.
