Write Complete Owner name as per Affection Plan with correct spelling

Provide if available

Select Construction NOC/Drawing Approval/Building Inspection as required


Write Complete detail regarding all building in project with all detail

Project Start Date and End Date

Write Percentage of completion


of project if available
Select Area name as per Affection Plan from Municipality

Provide correct Plot Number


For Ajman put Parcel ID Like 105352225
Provide Number of Blocks in that plot
Provide GIS coordinate in meters in WGS84 ZONE40 UTM system. Convert from
decimal degree to UMT.
Use decimal degree to UTM convertor. Many are available online and mobile
apps
PROVIDE
ALL
BUILDING
DETAIL
PROVIDE AREA OF SELECT
CONSTRUCTION AS LEADIN SIZE
PER DRAWING AS PER
DRAWING
PUT NUMBER OF DUCTS 50MM or
2 OR 4 AS PER DRAWING 100MM

• Affection Plan • Affection Plan with company stamp
• Telecom Drawing
• Architecture Drawing
For Building Inspection
• Application Form
• Municipality Affection Plan
• Building Permit
• Etisalat Approved Drawing
• Warranty Letter.
• Photos From the site as required per project.
• OTDR Report
• Owner under taking letter (only fill owner/authorized person detail
and sign)
