AFFIDAVIT OF BUSINESS CLOSURE


I, Ariel I. Malunes, Filipino, of legal age, and a resident of Brgy.
Estefania, Bacolod City, Philippines, after having sworn to in accordance
with law, hereby depose and say:

1. That I am the sole proprietor of UNO DEO PHARMA


DISTRIBUTOR which in 2013 I processed for a business
permit/license with the City of Bacolod;

2. On account however of our difficulty to hire a devoted


Pharmacist that is required by law, I decided to cease operating
and abandon the business in May 5, 2014;

3. That I execute this affidavit to attest the fact of business closure


of UNO DEO PHARMA DISTRIBUTOR registered in my name;

this 28th of September 2016 at Bacolod City, Philippines.
