I, Mr. RAMAKANT alias SHAMBHU PREMSAGAR GUPTA, age about


42 years, Hindu, Indian Inhabitant of Mumbai, residing at :
Panchganga Co-operative Society, Building 9/B, Room no-18,
Lallubhai Compound, Mankhurd, Mumbai-400043., do hereby
solemnly affirm and declare as under :-

I further say that my mother Mrs SHAKUNTALA PREMSAGAR


GUPTA expired on 12/06/2006 (Vide Death Certificate no
06ME0005518 ) and Father had Expired Prior to my Mother.

I say that I am the only one Legal Heir of Mrs SHAKUNTALA


PREMSAGAR GUPTA and nobody else can Claim Legal Heir
right other then myself.

I hereby indemnify the Concerned authority ie Registered of


Stamps, Kurla, Mumbai for any adverse claim by anybody in future
than I will be fully responsible for the same.

Whatever stated hereinabove is true and correct to the best of


my knowledge / information and belief.
