swelling tongue, itching, what would be your action as a nurse*
Stop the drug immediately
Check vital signs
Administer steroidal drug example Hydrocortisone to relieve itching
Administer epinephrine to reduce tongue swelling
Inform the charge nurse& physician/drug prescriber
Check patient file for history of previous allergic reaction and if any plan of action
has been documented to cater for such allergy
Document this reaction in patient file
Continue to monitor the patient

*8. You have a diabetic patient on the ward who is feeling anxious
and agitated, and uncomfortable and on 2mmol of insulin, what
would be ur action as a nurse*
1. assess the patient to get the actual cause of the problem by checking the blood
glucose level for this pt to ascertain if it’s normal or abnormal.
2. Manage as per your finding, if it is low administer or give a fast acting CHO and
if high u can give an oral glyceamic, inform ur line manager ( not sure abt this
though), administer prescribed iv fluid.
3. Change the patients position into a comfortable position, effective ventilation
and access for Pain .
4. Relieve anxiety - this might be as a result of the patients condition. Explain the
condition to the patient
5. Speak to the patient on the possible outcomes and the successful rate of
people with Diabetes and how it was properly managed .
6. Also diversional therapy can help.
7. Recheck blood sugar levels 15 minutes after treatment.

SpO2 of 85% and also complains of breathlessness?
The COPD patient.
Intervene immediately
Elevate head of bed to 45 degree
Check vital signs and oxygen saturation (SpO2)
Assess Airway, Breathing and Circulation.
Check there is no object obstructing airflow
Check for breathing, give Oxygen
Ensure adequate circulation
Inform ward manager/ physician
Continue to monitor patient
Document in patient record.

remove patient from the scene if it is unsafe
Assess the patient – check the vital signs, see if the patient is responsive
Call for help
Check for injuries or hemorrhage
If patient is conscious, Follow the Moving and Handling protocols and procedures
Stabilise the patient and place patient in a recovery position on the bed.
If patient is unconscious, initiate CPR until help arrives
Give oxygen, fluids and medications as prescribed
Monitor vital signs quarter hourly until patient is stabilize
Do a risk assessment

speaking to him in an abusive manner. What will be your action?
- raise safeguarding alert
3. I will get the Health assistant away from the patient and ask her to wait for me
in the staff room.
I will ensure the patient is ok ,Also reassure and calm the patient.
I will talk to the healthcare assistant about her behaviour, that it's not right and
patient's are meant to be treated with respect and dignity as well safeguarding
them against any form of abuse.
I will write report on the incident.
I will inform my line manager (even if that is the first time - so as to monitor the
healthcare assistant and prevent further reoccurrence, also in case of further
training on patient safeguarding measures.
I will follow the hospital policy and procedure on abuse.
