AFFIDAVIT OF GUARDIANSHIP

I, MARY ANN ICBAN NOLASCO, of legal age, Filipino citizen, married and residing at
Simbaluca, Sta. Teresita, Cagayan, after being first sworn according to law, hereby
depose and say, :

1. That I am the mother of JEROME I. NOLASCO born in Simbaluca, Sta. Teresita,


Cagayan on July 28, 1996;

2. That my son is intending to contract marriage with WEENA V. PALMA born in


Simpatuyo, Sta Teresita Cagayan, on February 5, 1992;

3. That I am competent to certify in behalf of the father of JEROME I. NOLASCO


giving the consent to marry WEENA V. PALMA;

4. That I am executing this affidavit for the purpose of informing the authorities
concerned of the veracity of the foregoing facts for whatever legal purposes it
may serve.
