Affidavit of Loss-Dti Certificate

Rosemarie C. Unida executed an affidavit of loss to attest that the original copy of her son Eugene C. Unida's certificate of business name registration for his fashion business "CHONA FOR MEN & WOMEN FASHION DESIGNER" registered with the Department of Trade and Industry (DTI) has been lost. AFFIDAVIT OF LOSS

I, ROSEMARIE C. UNIDA, of legal age, Filipino, married, presently residing at Centro


East, Sta. Teresita, Cagayan, after being duly sworn, hereby depose and state:
1. That I am the mother of Eugene C. Unida;
2. That Eugene C. Unida is the registered owner of a business under the trade name
CHONA FOR MEN & WOMEN FASHION DESIGNER which is registered
with the Department of Trade and Industry (DTI);
3. That before my son Eugene C. Unida went abroad, he trusted me to run the said
business and before he finally left he gave to me all the documents of the said
business including the original copy of the Certificate of Business Name
Registration issued by the Department of Trade and Industry (DTI);
4. That I placed the said original copy of the Certificate of Business Name
Registration together with other important documents inside a brown envelope
and keep it inside a cabinet;
5. That sometime on December 2020, I searched the said brown envelope
containing the original copy of the DTI Certificate of Business Name
Registration of the above-mentioned business but I am surprised because I could
no longer find the original copy of the Certificate of Business Name Registration
inside the envelope;
6. That despite diligent search and efforts to locate the said original copy of the DTI
Certificate of Business Name Registration, I could not find the same such that I
now believe that it is now lost beyond recovery;

7. That I am executing this affidavit to attest to the truth of the foregoing facts and
to use the same for whatever legal purpose it may serve.
