Workers were observed not following fall protection requirements during a scaffolding dismantling activity at a 700MW CSP + 250MW PV hybrid power plant project. An investigation found poor planning, inadequate training, and incompetent supervision contributed to the unsafe work. Lessons learned include the need to properly identify hazards, communicate risks, enforce safety procedures, provide pre-work training, and ensure close HSE and management supervision especially for high-risk activities to prevent accidents from unsafe acts or conditions.

 LESSON LEARNED AND KEY MESSAGES:


• Identification of all possible hazards and risk evaluation
for all activities and ensure safety measures in place.
• Clear communication of risk evaluation and hazards.
• Enforce HSE rules and procedures strictly to avoid grave
accidents resulting due to ignorance.
• Pre-work training and effective toolbox talk prior to start
an activity should be delivered and make sure awareness
and safety measures are clearly understood by workers.
• Empowered and adequate HSE, management
supervision, especially for high risk activities.
• Continuous and close monitoring by HSE & management
personnel to immediately stop any unsafe acts or
conditions till safety measures are in place and followed.
• Zero tolerance for unsafe acts and unsafe conditions
whilst carrying out any activity.
