Safety Alert

A car cleaner left rear seats unlocked while cleaning a vehicle. During a drive home, heavy braking caused an unlocked rear seat to press a fire extinguisher lever, releasing dry powder and causing a loss of visibility. Root causes included an unlocked seat. Recommended actions are to discuss the incident in meetings, distribute the alert to contractors, post it on boards, and include in inductions to always secure seats and fire extinguisher locking pins.

HSE LEARNINGS

INCIDENT
When cleaning his vehicle the car cleaners left the second row back seats upright but
did not secure them in place. Whilst driving home, the driver braked and the rear seat
moved forward pressing against the extinguisher lever setting it off releasing a high
pressure cloud of dry powder into the vehicle causing loss of visibility and a bad
distraction.

ROOT CAUSES:
• Near miss due to the low speed of vehicle.

Lateral Learning
• Always ensure the seats are locked into position.
• Always ensure the extinguisher locking pin is in place and is not the plastic type
which can easily break.

Recommended Actions:
Discuss this alert in toolbox talks and HSE meetings and
 Distribute to all contractors
 Post on HSE notice boards
 Include in site HSE induction
