graduation in Chemical Engineering and Masters in

Environmental Engineering.

He have a keen interest in HSEQ Audits, training & development

Having more than 22 years of experience in plant operations

and HSE field in different organizations. Since last 8 years he is

• Work Permit is a formal


documented procedure

• It authorizes certain people to carry


out specific work within a specified
time

• It sets out the precautions required


to complete the work safely, based
on a risk assessment.

A massive explosion & Fire occurred on July 8, 1988

167 dies,
• 109 by inhaling smoke

• 14 while attempting to escape

• A few died of burns.

What is a permit to work used for?
A permit to work is used to identify the following
areas:

• To define Scope , time and duration of job/activity

• To identify the authorized persons to do the work

• To define role and responsibilities of all working


parties

• To ensure job/activities carried as per method


statement/work procedure

• To identify hazard and control specific to job/activity


based on risk assessment/JSA

• To define prior action before job, such as the isolation

of sources of energy and outlets


Where PTW is required
A permit to work is required when there is a potential for serious harm to
workers or the environment or for carrying out any maintenance activity
This includes work that involves:

• Maintenance activity (CM - PM)


• Shutdown activity
• Hot work
• Work on high-voltage electrical systems.
• Confined space entry
• Work on operational pipelines
• Excavating near buried services
• Work at height etc
Permit Issuer and Receiver

Issuer

Issuer must be a person who is authorized to issue a Permit

Receiver

Receiver must be a person who authorized

to receive permit and responsible for the

work being completed safely


05 Main section of PTW
A permit to work usually has five main sections:

Issue
• Description of the work to be carried out
• The date and time of issue
• Hazard associated with the job/activity
• Controls required, such as isolations & LOTO
• Specific PPE required for job
• Emergency procedures.
• Signature of the authorized person issuing PTW
Receipt
Signature of the competent persons accepting the
permit (the workers)
Permit To Work
Clearance
Signature of the competent persons stating that the area has been made
safe e.g. Work completed

Cancellation
Signature of the authorized person stating that
• The isolations have been removed
• The area has been accepted back and that the
equipment can be restarted

Extension
This section is included in some permit systems to
allows the authorizing manager to grant an extension to the
timescale of the permit

Generally, seven types of work permits are in use;

• Hot Work Permits

• Electrical Work Permits

• Excavation Work Permits

• Confined Space Entry Permit

• Vehicle Entry Permit

• Radiography Work Permit

• Cold work permit


Confined Space

Confined space (1910.46 B)

• Is large enough and so configured

that an employee can bodily enter

and perform assigned work; and

• Has limited or restricted means for

entry or exit

• Is not designed for continuous

employee occupancy

Hot work" means riveting, welding, flame cutting or other fire or spark-

producing operation
Work at Height
Work at Height (HSE.Gov.Uk)

• Work at height means work in any place where, if precautions were not

taken, a person could fall a distance liable to cause personal injury.
