Subject: Declaration for conducting assessment as per standard operating process


I, _________________________________, with Aadhar number (__________________)
declares that I have been briefed by EduVantage about the maintaining the integrity and
sanctity of the assessment. I have clearly understood the standard operating guidelines of
assessments and I, hereby, declare that I will not indulge in any misconduct of any sort
which may include but not limited to bribery, misbehavior etc.
Also, as per guidelines, I am associated with 4 or less Assessment Agency (AAs) as the
assessor.
Also, there is no conflict of interest & I am not working as instructor/trainer/training partner
in skilling ecosystem in any capacity at this moment.
If any evidence is found which is contrary to above declaration, then I am ready to face any
action which may include legal or criminal action
